L’Institut de veille sanitaire, régulièrement sollicité par les autorités sanitaires pour contribuer à l’investigation des syndromes collectifs, a fait le constat qu’un nombre non négligeable de ces phénomènes ne trouvait pas d’explication fondée sur un mécanisme simple d’interaction entre l’environnement physicochimique et les processus biologiques. Ce type de syndromes non spécifiques et théoriquement rapidement résolutifs, parmi lesquels on trouve les syndromes de bâtiments malsains, peuvent prendre une ampleur inattendue en raison notamment du contexte psychologique et social dans lequel ils interviennent.

C’est ainsi que le besoin d’un guide s’est fait sentir, afin de proposer aux autorités sanitaires une méthode permettant une prise en charge optimale de ce type de situation, où évaluation et gestion sont très intriquées.

Les éléments rassemblés dans ce guide visent à apporter une aide au diagnostic, en envisageant tous les facteurs associés à l’apparition des syndromes, les aspects environnementaux mais également psychosociaux. Il propose également aux experts scientifiques et techniques les grandes étapes d’une démarche pluridisciplinaire et concertée. Ses objectifs opérationnels sont les suivants :
• favoriser la coordination des intervenants ;
• faciliter l’investigation ;
• rationaliser le recours à la métrologie ;
• aider à réaliser une communication appropriée ;
• aboutir au partage d’une culture commune.

Guidelines for diagnosis and management of mass psychogenic illnesses

The French Institute for Public Health Surveillance is regularly required by Health authorities to participate in mass psychogenic illness investigations. The Institute noted that a certain number of these syndromes could not be explained by a simple interaction mechanism between the physicochemical environment and the biological process. This type of non-specific mass illness, including Sick Building Syndrome, that should fi nd a rapid outcome theoretically, can spread unexpectedly, partly due to the psychological and social context in which they occur.

This is why guidelines are thought to be useful, in order to provide the Health authorities with a method allowing an optimal management of this kind of situation, where assessment and management are closely linked.

The elements included in our guidelines aim to facilitate diagnosis, by showing all factors linked with the illness, environmental as well as psycho-social aspects. The guide also provides the main steps of a multidisciplinary and concerted approach for scientific and technical experts. It presents following operational objectives:
• facilitate the coordination of the actors;
• help investigation;
• rationalize the use of metrology;
• help preparing appropriate communication;
• succeed in sharing a common culture.
